Why your face is changing — and why diet won’t fix it.

Faces change with time, and not equally everywhere. The bone underneath shrinks slightly. The fat compartments that give the face its youthful contour migrate downward and lose volume — particularly in the temples, cheeks, and around the mouth. The ligaments that suspend the soft tissue stretch. The result reads as “tired”, “gaunt”, or simply “not how I look in my head”.

There are usually four things going on at once: bone resorption changes the underlying scaffolding; fat-pad migration and loss creates hollowing in specific zones; ligament laxity allows soft tissue to descend; and skin thinning amplifies all of it. Sleep, stress, weight loss, hormonal change, and ageing all contribute differently.

This is why diet, skincare, or “just eating more” rarely solves it. The volume isn’t on the surface — it’s structural. The real answer comes from understanding which of those four factors is contributing most to your appearance, and addressing that one specifically rather than treating the whole face generically.

Volume loss is rarely solved by a single treatment. Most plans combine two or three of the following, sequenced over weeks or months. Final selection happens in the consultation, where the dominant cause is identified.

Anatomical filler placement (cheek, temple, jawline)

Hyaluronic acid filler placed in deep planes by anatomical landmark — not chasing surface contours. The aim is structural support: lifting the cheek soft tissue back to where it sits in younger anatomy, restoring temple curvature without making the face look “done”, or reinforcing a softening jawline. Quantity matters less than placement precision.

Bio-stimulators (Sculptra and Radiesse)

Sculptra (poly-L-lactic acid) works by stimulating your own collagen production over a course of treatment — results build gradually over several months and may last over two years, making it particularly suited to diffuse facial thinning, temple hollowing, and a generally gaunt appearance. Radiesse (calcium hydroxyapatite) provides immediate structural support to the cheeks, jawline, and chin while stimulating collagen over time — typically lasting 12–18 months — and in selected patients can be hyperdiluted and used more superficially to improve skin laxity and crepey texture.

Profhilo Structura

Unlike traditional Profhilo, Structura is placed using a cannula technique and is designed to support the superficial fat compartments and tissue quality rather than delivering hydration alone. In carefully selected patients with early facial thinning or laxity, it helps improve skin quality, tissue support, and facial structure — a regenerative option for when volume loss is still in its early stages.

Sudden or rapid volume loss can occasionally signal something the consultation should investigate before any aesthetic treatment — thyroid changes, weight loss, medication effects, or other clinical factors.
