Texture has several overlapping causes, and the encouraging part is that most of them respond well to medically guided treatment, sequenced gradually — rather than another aggressive scrub or acid bought on guesswork.
What's actually going on
Texture is about the skin's architecture — not “bad skin”.
Smooth, healthy skin reflects light evenly. Uneven texture happens when the skin’s surface architecture and collagen framework are disrupted — enlarged pores, roughness, congestion, acne scarring and irregular collagen repair all scatter light, create shadowing, and make the surface read as uneven, especially under make-up or harsh lighting. It’s rarely “bad skin” or a skincare failure.
Several factors usually overlap: enlarged pores linked to oil production, collagen loss, congestion or previous acne; slowed cell turnover leaving the surface rough and dull; acne scarring and irregular collagen repair after inflammation; dehydration and a compromised barrier making every irregularity more visible; and sun damage, ageing and hormonal change quietly reducing collagen quality underneath it all.
Just as important: not every texture concern should be treated aggressively. Where the barrier is inflamed, dehydrated, reactive or over-exfoliated, more acids and harsher treatments make things worse, not better. The consultation works out whether pores, congestion, scarring, dehydration or collagen loss is the dominant issue — and sequences treatment gradually, sometimes starting with barrier repair before anything stronger.

Different peels work at different depths — and should be chosen for your barrier health, sensitivity and pigmentation risk, not by strength alone. The Obagi Radiance Peel is a more superficial option for brightening, mild roughness and glow with minimal downtime; the Perfect Peel is a stronger medical-grade treatment for uneven tone, superficial acne scarring and congestion, encouraging healthier turnover and collagen stimulation. Resurfacing is introduced gradually — over-aggressive peeling can worsen barrier dysfunction, sensitivity and pigmentation.

Polynucleotides & regenerative skin treatments
Regenerative injectables that improve skin quality, hydration, collagen support and tissue repair — particularly valuable when the barrier is too compromised for aggressive resurfacing initially, or where scarring and texture change coexist with inflammation, dehydration or fragility. Long-term texture improvement usually needs consistent medical-grade skincare alongside any procedure. Evidence-based ingredients — tretinoin for cell turnover and collagen stimulation, azelaic acid for congestion and post-inflammatory marks, gentle exfoliating acids, antioxidants and daily SPF — are fundamental for pore appearance, smoothness and long-term skin quality. Equally important is what to avoid: over-exfoliating or layering too many actives at once worsens inflammation and barrier dysfunction, especially in skin already struggling with texture.

Texture treatment starts with an honest read of the skin in front of us: barrier health, inflammation, acne history, pigmentation risk and sensitivity — before anything stronger is recommended. Some patients first need barrier repair, simplified skincare, acne control or inflammation management before resurfacing makes sense — and if that’s you, you’ll be told plainly, with a plan for getting there.
